Forgot your weekly weight-management injection? The safest next step depends on which medication you use, when the dose was due, and when your next dose is scheduled. Wegovy, Ozempic, Mounjaro, and Zepbound do not all follow the same missed-dose rule. This guide helps you identify the right instructions and recognize when to contact your prescriber or pharmacist.
Quick Answer: What Should You Do If You Miss a Dose?
Check the exact name and formulation on your prescription label, then follow that product’s missed-dose instructions. For weekly injections, some products allow a late dose within a specific window, while others require skipping it when the next dose is too close. Never take two doses together to make up for a missed injection. If you missed several weekly doses, ask your prescriber how to restart safely.
What Should You Check First After Missing a Dose?
- Confirm the product. Read the medication name and whether it is a weekly injection or daily tablet.
- Record your dates. Note when the dose was due, when you last actually took medication, and when the next dose is scheduled.
- Consult the product instructions. Use the medication guide or prescribing information for your exact product, not a general GLP-1 rule.
- Ask before restarting if uncertain. Especially if you missed several doses, have troublesome side effects, or take other medicines for diabetes.
If your medication was unavailable and that caused the interruption, see our guide to weight-management prescription delays and out-of-stock medications.
Missed-Dose Instructions: Wegovy, Ozempic, Mounjaro, and Zepbound
The following summary concerns U.S. labeled weekly injections. It is educational, not an individualized dosing instruction. Always check the instructions supplied with your prescription.
| Medication | If you missed one weekly injection | Important distinction |
|---|---|---|
| Wegovy injection (semaglutide) | If the next scheduled dose is more than 48 hours away, take the missed dose as soon as possible. If it is less than 48 hours away, skip and resume on your usual day. | If you missed two or more consecutive doses, contact your healthcare professional about restarting; re-escalation may be needed. |
| Ozempic injection (semaglutide) | Take the missed dose as soon as possible within 5 days of when it was due. If more than 5 days have passed, skip it and resume on your usual day. | Ozempic is labeled for specific diabetes-related indications; do not assume its instructions match Wegovy’s. |
| Mounjaro injection (tirzepatide) | Take the missed dose as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip and resume on your usual day. | Do not take two doses less than 72 hours apart. |
| Zepbound injection (tirzepatide) | Take the missed dose as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip and resume on your usual day. | Do not take two doses less than 72 hours apart. |
At an exact cutoff or if your dates are unclear: check the product’s current instructions or ask a pharmacist rather than guessing. These windows are not interchangeable. Official product labeling is listed below.
What If You Miss One, Two, or Three Weeks?
One missed weekly injection
Use the medication-specific timing rule above. Missing one injection does not mean you should take an extra dose the following week.
Two or more consecutive missed injections
Contact your prescriber before deciding how to restart. With Wegovy injection, current labeling recognizes that restarting with dose escalation may reduce gastrointestinal reactions after multiple missed doses. For other products, your clinician may consider the length of interruption, previous dose, treatment response, and tolerability. Do not invent a restart dose yourself.
Three weeks without an injection
Do not automatically resume a previously escalated dose. Record the date of your last injection and ask your prescriber for a restart plan. If a refill delay caused the gap, your pharmacist may help you clarify the prescription status and next steps.
Can Missing a Dose Cause Side Effects When You Restart?
Missing a dose may interrupt the intended treatment schedule. After a longer gap, restarting—particularly at a higher previously tolerated dose—may bring back or worsen gastrointestinal symptoms such as nausea, vomiting, or diarrhea. This is one reason a prescriber may reassess the restart plan rather than simply telling you to continue where you left off.

Can You Change Your Weekly Injection Day?
Some products permit a different weekly injection day, but minimum spacing matters:
- Wegovy injection: the last dose must have been at least 2 days earlier.
- Ozempic: the label permits a change when doses are more than 48 hours apart.
- Mounjaro and Zepbound: doses must be at least 3 days (72 hours) apart.
These rules do not authorize extra doses. If you are changing the day because of a missed injection, confirm the timing with your pharmacist.
What About Wegovy Tablets or Compounded Semaglutide?
Wegovy tablets are taken daily, not weekly
If a daily Wegovy tablet dose is missed, the labeled instruction is to skip that missed dose and take the next dose the following day. Do not apply the weekly injection’s 48-hour rule to tablets.
Compounded semaglutide or tirzepatide
Compounded preparations are not the same as FDA-approved brand products. Do not assume that a compounded vial has the same concentration, administration schedule, or missed-dose instructions as Wegovy, Ozempic, Mounjaro, or Zepbound. Follow the individualized instructions from your prescriber and dispensing pharmacy; contact them if the instructions are unclear.
How Can You Reduce the Chance of Missing Another Dose?
- Set a recurring phone reminder for the prescribed day.
- Keep a simple record of the actual date you take each dose.
- Check your supply before your next scheduled injection; request refills with enough time for processing.
- Ask your pharmacist whether the product needs special storage or travel precautions.
- If you expect a gap because of coverage or cost, review our weight-management medication insurance coverage guide.
Frequently Asked Questions
1. What should I do if I miss a semaglutide injection?
First identify whether your prescription is Wegovy injection or Ozempic injection. Their labeled missed-dose windows differ. Use the instructions for your exact product and contact your pharmacist if you are unsure.
2. Can I take Wegovy injection one day late?
For a single missed Wegovy injection, labeling says to take it as soon as possible if your next scheduled dose is more than 48 hours away. If it is less than 48 hours away, skip the missed dose. Confirm your schedule if the timing is borderline.
3. What if I miss an Ozempic dose by six days?
Ozempic labeling says to skip a missed dose when more than five days have passed and take the next dose on the regularly scheduled day. Do not take an extra dose to compensate.
4. How late can I take Mounjaro?
Mounjaro labeling allows a missed weekly dose within four days (96 hours) after the scheduled time. After that, skip it and resume the regular schedule. Keep at least 72 hours between doses.
5. What if I miss Zepbound for one week?
Use Zepbound’s four-day missed-dose window. If that window has passed, skip the missed dose and follow the normal schedule. If you have missed multiple injections or are uncertain about restarting, contact your prescriber.
6. Can I restart Zepbound after missing three weeks?
Ask your prescriber for an individualized restart plan before resuming a previously escalated dose. Do not choose a restart dose based on another patient’s experience.
7. Can missing a GLP-1 dose cause nausea when I restart?
Gastrointestinal symptoms may occur when treatment is restarted after an interruption, particularly if the dose needs reassessment. Ask your prescriber about safe reinitiation rather than adjusting your dose yourself.
8. Should I take two injections to catch up?
No. Do not double a dose to make up for a missed weekly injection. Follow the labeled timing rules for your medication and ask a pharmacist if you are uncertain.
9. What if I miss one day of a Wegovy pill?
Wegovy tablets have a daily schedule. The labeled instruction is to skip the missed tablet and take the next dose the following day. The injection instructions do not apply to the tablet.
10. What if I miss a dose because my pharmacy is out of stock?
Contact your pharmacist and prescriber promptly. Ask about availability and whether the interruption changes your restart plan. Do not substitute a different medication or strength without prescriber authorization.
